The results of this meta-analysis support the assertion that crack-cocaine users have significantly greater risk of acquiring HIV and hepatitis C compared to users of powder cocaine. Rates of syphilis, HCV, and HIV were also statistically higher among crack-cocaine users compared to powder-cocaine users. The findings of the meta-analyses support calls to improve access to substance abuse prevention, testing, and harm reduction services, because overall levels of self-reported syphilis, HCV and HIV were significantly higher among crack users.
The new study’s findings “suggested a stronger association with crack use, probably because crack is more commonly used by those with lower socioeconomic status,” Palamar added. That result “likely influenced the perception that health outcomes were more severe for crack users, and appeared to be consistent with the data from this study.” He said a future study is needed to understand why the racial disparity persists.
The argument against crack legalization is that it will lead to higher rates of violence, crime, and unemployment, through both the introduction of cash-crop farming and the diversion of resources from policing and preventive health programs. But there is very little evidence to support this contention.
The faisble association between crack use and blood-borne viral infections is of real concern in light of the fact that most of these infections are preventable through the use of universal precautions, which include the avoidance of injection and needle sharing. This finding is consistent with data from most HIV surveillance programs.
Crack is more likely to be addictive than cocaine. It produces similar effects in the body, but does not carry the same risk of toxicity. The harmful effects of crack are immediate, and severe, while those of cocaine can develop hours or even days after the initial exposure. Cracks are made in small pipes or “bongs”, usually of plastic or glass, and it is a common practice to cook them (i.e. make them hot) to produce more of a “kick”. The percentage of cocaine-to-crack is generally 10:1, but the percentage is not recorded for most people who try crack. The higher crack-to-cocaine ratio is because the finished product is the most popular method of smoking crack. It appears to be less expensive to produce than cocaine, and the drug does not have the restrictions on handling and sales (for example, the illegal sales and the punishments) that cocaine has.
Sometimes referred to as “the liquor” or “the drug of choice” among substance abusers, crack is often brought into treatment as a first step to treatment. The treatment setting is often the first point of entry into the addiction cycle.
Addiction treatment for drug users is challenging for many reasons. Some clients have significant mental health problems that they will need to address before a complete recovery can be achieved. They are also heavy users of prescription medications for medical problems, which have the potential to be co-ingested with the illegal substances. Individuals who use crack cocaine have particular difficulties in terms of providing insight into the addiction because of the immediate effects of the drug. Patients who are long-term crack abusers, however, are typically able to describe the pattern and hold on to a particular experience that caused them to want to keep using. However, these patients may not be able to fully identify the reasons that made them want to use the drug.
